Physical Paperwork
Any student planning to participate in Athletics, JROTC, or Band for the 2023 – 2024 school year must have a completed physical on file by their first practice.
As a convenience, Central Catholic has a physical form below to be completed by a preferred physician. Families can also submit a standard physical form generated by a physician's office.
