Return Forms At Once

Certain forms must be filled out and submitted to the front desk (Attendance Office) at Central Catholic prior to all students' matriculation or return to Central Catholic. The first form (Student-Parent Handbook Acknowledgement and Acceptance) is required, along with immunization records, from all students. The JROTC forms are required for all JROTC students, including all freshmen and sophomores. The medical emergency form is required from all students.
Students who do not have forms in by August 22 will be sent home. There are serious liability issues for the school if we do not have all these forms on file. Thank you for your understanding.
The form affirming that parents and students have reviewed the Student-Parent Handbook is required and was issued with the Handbook on July 17, 6:30 p.m., at all-class orientation evening. Thank you for helping us do our job by filling out the appropriate forms. They are all in open (.pdf) format. If you have a word processor that can handle pdf files, you may fill them out on your computer. Otherwise, please print them out and fill them out as legibly as possible. Print is usually easy for us to read.
