End-of-Year Medication Pick-Up and 2026–2027 Health Forms
Dear Students and Parents,
As we wrap up another successful school year, this is a friendly reminder that all student medications currently stored in the Nurse’s Office must be picked up by the end of the school day on Monday, June 1, 2026. Per the CCHS Student/Parent Handbook, any medications not picked up by this deadline will be properly disposed of according to school protocol.
Please make arrangements to:
- Pick up your student’s medication on or before June 1, 2026, or
- Contact me prior to June 1 if you would like to discuss arrangements for medications to remain secured in the Nurse’s Office for the upcoming school year.
Required Health Documents for the 2026–2027 School Year
Please complete and submit the appropriate forms before the start of the new school year. Links to medication consent forms and the student health history form are provided below. Completed forms may be emailed directly to me or returned to the Nurse’s Office at the beginning of the school year.
Required Prior to the Beginning of the School Year
- Immunization records for all incoming freshmen
- Updated immunization records for current students
- Signed medication consent forms for all prescription and over-the-counter medications
- Current action plans for chronic health conditions, such as:
- Asthma
- Diabetes
- Seizure Disorders
- Migraines
- Severe Allergies
- Updated health information, including any chronic conditions or recent health changes
- Documentation of all allergies (food, medication, insect, or environmental) and any required emergency medications (e.g., epinephrine auto-injectors)
