As we kick off the new school year, please make sure the health office has the most up-to-date information to help keep every student safe and healthy.
Reminders:
- Updated Immunization Record – required for all students.
- Incoming Freshman/Transfer Students – must submit a complete immunization record before starting school or no later than August 28,2026.
- Over-the-Counter Medication Consent Form – filled out and signed by a parent/guardian.
- Daily Prescription Medication Consent Form – filled out and signed by a physician.
- Allergies – list all allergies and note the degree of reaction.
- Action Plans for Chronic Conditions – such as asthma, diabetes, allergic reactions, etc., must be on file.
