Physical Examinations offered at Central Catholic

on Monday, May 18,2026

Physical Examination are being offered in the KCCC for the 2026-2027 school year and are scheduled for Monday, May 18, 2026, at 4:00 pm. The cost is $20.

Everyone receiving a physical must have a completed PPE package completely filled out and signed by both the student and parent/guardian turned into Lanning Tucker, MSS.ACT/LAT by Monday, May 11th.  This will ensure that Methodist Sports Medicine, SMASA, and AugustHeart have an accurate roster that is ready for the examinations.

Paper copies of the PPE package are available in the KCCC and can be obtained from Lanning Tucker however, if you choose to, you may print out the forms found at the bottom of this page, complete all sections and sign, and email the fully completed forms to Lanning as a PDF file. JPEG files are not acceptable as the Doctors will be filling out the physical examination:

Physicals will begin right at 4:00 PM and everyone on the list to receive a physical examination MUST arrive and check-in no later than 5:00 PM.

With the exception of Incoming Freshmen, height, weight, blood pressure, and visual acuity measurements must be done before the physical examinations begin. There will be signs and assistance to direct students from the check-in tables to each station as follows:

Key points to remember.

  1. Athletes must have a completed and signed medical history form (signed by the student and parent) in addition to the physical exam form for the doctor providing the PPE the day of the event.
  2. A passing blood pressure reading is under 140/90.  Any BP notated on the form that is above 140 on the top number or 90 on the bottom will be flagged for follow up with the athlete's personal physician..  Pulse must be 99 or below.  All height, weight and blood pressure must be taken before the day of the physicals (except for incoming Freshmen).
  3. The AugustHeart EKG screen is specifically looking for issues related to sudden cardiac death in athletics.  This screening does not look at issues related to cardiac issues from COVID infections as those types of issues are not picked up with EKGs but require in-office diagnostic scans. AugustHeart is for current 8th through 12th grade students.
Parents/Guardian may also have their son's physical examination done by their Primary Care Physician but it is recommended that the TAPPS Medical Packet below be used for off-site examinations as well.

After completed, please email any/all physical forms as a .pdf file to Lanning Tucker, MSS, ATC/LAT at [email protected]. Do not turn them in to Coaches or JROTC Instructors so as not to delay your son's physical or any medical forms being uploaded.

Please email or call Lanning at (210) 225-6794, extension 313 if you have any questions.
