Re-Enrollment Form

In order to re-enroll your child, this form must be completed and returned to our Business Office with the $200 enrollment fee by April 7th. Please note that parent/guardian signatures are required on this form.
Scholarship Information Form- SENIORS ONLY

Seniors must complete the attached form listing all scholarships that the student has been offered. A copy of each awarded scholarship MUST also be turned in with this form to your respective counselor. This affects any calculations, ceremonies and/or printed materials pertaining to graduation. The deadline to submit this information is April 21, 2017.
College Acceptance Form- SENIORS ONLY

Seniors must submit a copy of each acceptance letter with this form to your respective counselor no later than April 21, 2017. This pertains to the acceptances placed in any announcements or literature pertaining to graduation.
College Visit Permission Form

If your son is a junior or senior and is beginning the search for the university of his choice, he will likely be visiting various campuses around the country. Prior to his schedule college visit he will need to print and complete the permission form and submit it to his counselor 2 days prior to the visit for approval.
Transcript Requests Forms

Transcript Request forms for current students are available in the Counseling Office, rm. 334.
